Community Health Needs Assessment

Southern Coos Hospital & Health Center is in the midst of conducting a Community Health Needs Assessment. A Community Health Needs Assessment (CHNA) is a systematic process involving the community to identify and analyze community health needs. The process provides a way for communities to prioritize health needs, and to plan and act upon unmet community health needs. Part of completing a quality needs assessment is to survey the community to better understand individual perspectives on health and health needs.

They are officially live with their community health needs assessment (CHNA) survey and really trying to engage the community better this time around so that we have really good data about health and health needs in the community.

The survey should be completed by anyone over the age 18 that lives and/or works in the Bandon, Port Orford, Langlois or surrounding areas, and unlike the census should/could be done by every adult in the house (or business) rather than just one.

You can complete the survey on line, by requesting a paper survey be mailed to you, in person, or over the phone:
